The Community Office can assist you with issues pertaining to provincial services and programs. If required, staff can also help connect you with local community resources and refer you to other levels of governement.

The services our Community Office Team can assist with include, but are not limited to:

    • The Workplace Safety & Insurance Act governs the Workplace Safety & Insurance Board (WSIB): This Act requires that any person acting as a representative for an injured worker be either: a lawyer, a paralegal, a representative of a trade union, or a member of the Office of the Worker Advisor.
    • *Note: MPP office staff are not fuly trained to represent workers, but may be able to assist you with finding the representation you deserve.

MPP West presents, debates, and votes on proposed laws (bills) in the House (Queen's Park). In committees, he studies bills, issues, or topics, and reports back to the House.

The constituency office staff are authorized by the Province of Ontario to take affidavits and administer oaths, affirmations, or declarations.  Please see the below information on how they can support you in this role. 


What is a Commissioner for Taking Affidavits?
A Commissioner for Taking Affidavits is a person who is authorized by the Province of Ontario to take affidavits and administer oaths, affirmations, or declarations. Commissioners are governed by the Commissioners for Taking Affidavits Act, 1990.


Fee - $0
The office does not charge a commissioning fee. However, the staff are bound by their responsibilities as a Commissioner for Taking Affidavits and have been authorized to refuse to commission any document at their discretion.

Note: If a Commissioner is unsure about the identity of the person or the content of the document for any reason, the Commissioner may refuse to sign the document.


We do not provide the services of a Notary Public.
A Notary Public is a person authorized by the Notaries Act to commission documents, certify or notarize documents as true copies, and to verify signatures. The requirements to become a Notary Public are stricter than for a commissioner and a Commissioner cannot sign on behalf of a Notary Public.


Documents that can be signed:
The staff that have been designated to serve as Commissioners for Taking Affidavits can sign documents at their discretion, including but not limited to the following:
• Proof of residency.
• Transfer of vehicle ownership.
• Pension plan and insurance claim documents.
• Statutory declarations on government-related forms.
• OSAP related documents.
• Travel letters and permissions (if not otherwise specified).


Documents that cannot be signed:
The Commissioner for Taking Affidavits at the office are not authorized to sign documents related to:
• Any forms related to a medical matter.
• Wills and testaments.
• Powers of Attorney.
• Divorce, separation, and custody.
• Real estate.
• Court documents or any documents related to court proceedings.
• Any other documents requiring a Notary Public.


Before you start:
Please contact the office in advance. Our team can help to confirm if your documents can be signed by a Commissioner for Taking Affidavits and can also confirm when a Commissioner from the office is available.

You will need to bring valid photo identification. The identification must show your photo, your full name, and signature.

X Do not sign the document before arriving. The document(s) to be commissioned must be signed in the presence of a Commissioner of Oaths.

Before the Commissioner for Taking Affidavits can sign your document(s), they must be completed in full. You can complete this at home or at the MPP office. Staff cannot commission a blank or incomplete form.


Your responsibility
If you are signing a legal document that requires your oath, affirmation, or declaration, the content of that document is your sole responsibility. When you are making an oath, you are known as the "deponent" –the person whose signature is being commissioned to ensure the content of the affidavit. As a deponent, you must understand the details in the document to which you are attesting– and you must understand that, by swearing an oath, you are attesting that those details are correct.

MPP West is happy to provide greetings for events, certificates of achievement or recognition for individuals and organizations, and celebratory messages for milestone birthdays, anniversaries, and announcements.

  • Full name of the person requesting the scroll, phone number, email
  • Full name of the recipient(s)
  • Scroll occasion/milestone being celebrated (ex. birthday, anniversary, etc...)
  • Date of occasion/milestone being celebrated
  • Date the completed scroll is required
  • Mailing address or indicate if the scroll will be picked up at our community office
  • Please put "Special Request" in the subject field to ensure it is received correctly

Should staff be able to fulfill your request, our office will confirm the above details with you.

While MPP West endeavours to attend as many events as possible, please note that due to his busy schedule, he is not able to accept every invitation. If he is able to attend, our office will contact you to confirm details.
